1. What this site is

Cleveland Health Bridge ("we," "us," "the site") is a free directory that helps people find social services and community resources in Greater Cleveland and Cuyahoga County, Ohio. It is a community project created and maintained by volunteers, including medical students. It is provided for general informational purposes only.

2. Not medical, legal, or financial advice

Nothing on this site is medical advice, diagnosis, or treatment, or legal, financial, insurance, or benefits advice. Nothing here creates a doctor-patient, attorney-client, caseworker-client, or any other professional relationship between you and us or anyone who contributes to this site. Do not delay or skip getting professional care because of something you read here. Any guide, checklist, or tool on this site, including anything about Medicaid or other benefits, is only a general aid. It does not decide whether you qualify for anything. Only the agency that runs a program can decide eligibility.

3. We do not run or control the listed services

Every organization, program, clinic, pharmacy, shelter, and service listed on this site is independent. We do not own, operate, manage, supervise, or control them. We are not responsible or liable for:

  • the services, care, goods, or help any listed organization provides or fails to provide
  • the quality, safety, legality, or availability of those services
  • who a listed organization accepts or turns away, or what it charges
  • how any listed organization or its staff or volunteers treat you
  • anything that happens to you at, during, or because of your contact with a listed organization

Any dealings you have with a listed organization are between you and that organization only.

4. No endorsement, recommendation, or guarantee

Being listed on this site does not mean we endorse, recommend, certify, license, inspect, or vouch for an organization. We do not guarantee that any listed organization is safe, reputable, open, accepting new clients, able to help you, or right for your situation. You are responsible for deciding whether a service is right for you.

5. Accuracy and how we try to keep information current

We make a real effort to keep listings accurate and up to date. We start from trusted public sources, such as the NEOCH Cuyahoga County Street Card, and we update listings as we learn of changes, including through reports from users and volunteers. Even so, services open, close, move, change hours, change phone numbers, and change who qualifies, often without notice. We cannot promise that any listing is complete, correct, or current. Information may be missing, outdated, or wrong. Always call or check directly with the organization before you go, and have a backup plan.

6. No warranties

This site and everything on it are provided "as is" and "as available," without warranties of any kind, whether express or implied. This includes any warranty of accuracy, completeness, reliability, availability, or fitness for a particular purpose. We do not promise the site will always be available, error-free, or secure.

7. Limitation of liability

To the fullest extent allowed by law, Cleveland Health Bridge, [LEGAL ENTITY NAME, once formed], and its founders, volunteers, contributors, students, faculty advisors, officers, board members, and affiliated institutions will not be liable for any direct, indirect, incidental, consequential, special, or other loss, harm, injury, or damages of any kind arising from or related to your use of, or inability to use, this site; your reliance on any information on it; or your contact or dealings with any listed organization. This applies even if we were told such harm was possible. Some laws do not allow certain limits, so some of this may not apply to you.

8. Use at your own risk

You use this site and any information on it at your own risk and by your own choice. You are responsible for checking information before you rely on it and for decisions you make about your health, safety, housing, and finances.

9. Emergencies

This site is not for emergencies and cannot respond to emergencies. If you or someone else is in immediate danger or having a medical emergency, call 911. For a mental health or suicide crisis, call or text 988. For help finding services in Ohio, you can also contact 211.

11. User reports and suggestions

You can report a problem or suggest a resource. We review submissions but are not required to publish or act on them, and we cannot verify every submission. By submitting, you confirm that the information is given in good faith. Please do not include private health or personal information in a report or suggestion.

12. Signed-in users and volunteer portal

Some features are for approved volunteers. Do not post private health information or identifying details about patients or clients in the volunteer portal, timeline, or any form on this site. Volunteers act in their own capacity and are not providing medical care through this site.

13. Privacy and usage data

We do not track anonymous visitors with our own usage system. Signed-in users may choose to allow usage tracking; this is optional and based on consent. Our website host may still count page views in aggregate. Read the privacy policy.

14. Data sources and licenses

Listing information comes from public sources, including the NEOCH Cuyahoga County Street Card (Spring 2025, licensed CC BY 4.0) and Ohio Department of Developmental Disabilities data, along with community contributions. Site data is shared under CC BY 4.0 and site code under the MIT License. Use of third-party names is for identification only. Our use of a source does not mean that source reviewed, approved, or endorses this site.

15. No official affiliation or endorsement

Cleveland Health Bridge is a community project. It is not an Ohio University program or a Cleveland Clinic program, and it is not affiliated with either one in any way. It has not been reviewed or endorsed by Ohio University, the Cleveland Clinic, or any listed organization. Views and content on this site are those of the project and do not represent any institution.

16. Changes to this page and the site

17. Governing law and severability

These disclosures are governed by the laws of the State of Ohio. If any part of them is found unenforceable, the rest stays in effect.
